Supported Living Services

SLS offers a variety of individualized and flexible supports for adults in and outside of the home that cannot be met by other means.  Services enable adults to live in their own homes or in family homes and avoid or delay the need for more costly comprehensive services.  The level of support and supervision provided varies based on the individual’s needs as identified in the individualized plan.  Supported living services aim at augmenting already available supports for adults who either can live independently with limited supports or who, if they need extensive support, are principally supported from other sources, such as their family.  These supports may include:

·         personal assistance

·         employment

·         community integration services

·         assistance with decision-making problems like money management

·         assistive technology

·         home modification

·         professional therapies

·         transportation

Comprehensive Services

These services are utilized to meet the needs of adults with developmental disabilities who cannot live safely without substantial supports and supervision and for whom only limited outside supports or no other appropriate options are available.  Persons in Comprehensive Services have or receive access to 24-hour supervision and other supports, training and habilitation based on an individualized plan.  Comprehensive Services include:

·         individualized residential services and supports in a variety of community-based settings

·         employment or other day services

·         transportation
