Eligibility

How do I know if I am eligible?

Persons with a developmental disability are those having a "disability that is manifested before the person reaches twenty-two years of age, which constitutes a substantial disability to the affected individual, and is attributable to mental retardation or related conditions which include cerebral palsy, epilepsy, autism or other neurological conditions when such conditions result in impairment of general intellectual functioning or adaptive behavior similar to that of a person with mental retardation" (CRS 27-10.5-102). For children under five years of age, eligibility is based on determination of either a developmental delay or factors putting the child at risk of having a developmental disability. Additionally, the eligible individual must show residency in Colorado either by residing in Colorado at the time of application or exhibiting the ownership of property in Colorado by the individual and/or family.

In Colorado, a person must be determined eligible by a Community Centered Board. Community Connections, Inc. is the Community Centered Board that serves Archuleta, Dolores, La Plata, Montezuma and San Juan counties in Southwest Colorado. Once determined eligible by a Community Centered Board, an individual can take that eligibility with them anywhere they live in the State of Colorado.

Other specific eligibility requirements may apply to certain programs within the DD system. Some programs require functional eligibility for Medicaid and/or a determination of disability through the Social Security Administration. Many of the programs at Community Connections, particularly the adult programs that include Supported Living Services and Comprehensive (Residential) Services, have waiting lists. Some programs require that the individual be placed on a waiting list at the statewide level. Contact Community Connections if you have questions about waiting lists.
